Job Description

This position is responsible to provide outreach, engagement, and intervention services to individuals seeking access to recovery and treatment services; engagement of individuals and families appropriately to navigate treatment, social services, and recovery support systems; and assisting individuals with urgent needs in accessing services to support recovery, wellness, and healthy living. The Community Health Outreach Worker works collaboratively to prevent harm, support overdose prevention, coordination of recovery and treatment services, and provide individual- and group-level interventions to reduce risk of disease transmission and increase access to care and treatment.
Position Outcomes:
Provide Peer Recovery Support to individuals through mentorship and ongoing support to individuals dealing with substance use disorders and connect them with the resources that can support a pathway to recovery.
Engage individuals who have survived an overdose and their families to provide assistance and recovery support.
Provide harm reduction services to individuals, families, and community members.
Provide outreach to communities on the White Earth Reservation for substance misuse prevention and intervention.
Engage and support individuals and families affected by substance misuse and abuse, including during, or immediately following crisis situations.
Provide critical peer recovery support and intervention services to enhance post-treatment or post-incarceration recovery outcomes.
Maintain proper documentation standards according to professional licensure requirements, clinical billing processes, and all internal policies and procedures.
Communicates with internal and external sources to ensure program and organization is represented in a professional manner.
Knowledge, Skills, and Abilities:
Knowledge of ASAM Level of Care and other modalities of treatment and recovery.
Knowledge of 12 Core Functions according to licensure/certificate.
Knowledge of DANNES and other data reporting standards.
Knowledge of federal laws including Health Insurance Portability Accountability Act (HIPAA).
Ability to write reports and business correspondence.
Ability to provide strengths-based, individual-centered proactive harm reduction services.
Ability to build rapport and working relationships with cultural and ceremonial providers.
Familiarity with and ability to access community resources and services.
Qualifications:
High School Diploma, or equivalent (G.E.D.)
UMICAD Level I preferred.
One (1) year or more in recovery from substance/opioid use disorder.
Two (2) years’ experience with outreach, case management or direct service with individuals with history of mental illness, homelessness, and/or substance/opioid use disorder.
Have peer recovery Support certification, or ability to obtain within 60 days of hire.
Ability to complete Rule 25/Comprehensive Assessment certification within 6 months of hire.
Ability to work flexible hours including evenings and weekends.
Cultural knowledge and ability to apply it to individual and group work.
Ability to respond to crises after business hours and weekends as part of regular on-call scheduling.
Ability to work in dangerous situations or environments, communities including crisis situations involving substance misuse.
Valid Driver’s license and insurance for travel between sites and other business-related destinations.
