This position will provide mental health targeted case management (MH-TCM) services to adults with a serious and persistent mental illness (SPMI) and children with a severe emotional disturbance (SED). The case manager will assess the client’s changing needs, develop a plan with the client and the client’s other services providers to best address the client’s needs, link the client to appropriate community resources and monitor the effectiveness of planned services. The case manager will provide activities that are coordinated with community support services and are designed to help individuals who qualify obtain needed mental health services, social services, educational services, health services vocational services, recreational services, and related services in the areas of volunteer services, advocacy, transportation, and legal services.
Position Outcomes:
- Under the direction of a designated clinical supervisor, provide case management services (information, plan referral and linkages) to clients on her/his assigned caseload.
- Engage with clients from an empowerment, strength-based perspective and promote client self-determination and develop trusting, respectful relationships with clients.
- Complete a written functional assessment and develop the Individual Family and Community Support Plan (IFCSP) for children and the Individual Community Support Plan (ICSP) for adults based on the diagnostic assessment and functional assessment.
- Coordinate and advocate for services with other professionals and paraprofessionals.
- Arrange for transportation of clients to appointments, community agencies, etc., as needed.
Knowledge, Skills, and Abilities:
- Knowledge of demographics, culture, customs, and geography of the White Earth Reservation.
- Knowledge of Mental Health targeted case management services with an understanding of local community resources and how to utilize these resources to meet client needs.
- Knowledge and understanding of the clinical work done in a community mental health program.
- Skilled in identifying and resolving problems independently in a variety of settings with both clients and professionals within the community.
- Ability to establish and maintain positive rapport with a wide variety of clients and professionals.
- Proven ability to work independently, take initiative, identify strengths in individuals and systems and positively persuade and motivate others.
- Skill in personal computer and related software such as Microsoft Office (Word, Excel, Outlook), and internet.
Qualifications:
- Bachelor’s Degree in one of the behavioral sciences or related fields, including but not limited to social work, psychology, or nursing from an accredited college or university or a minimum of 3 years’ experience as a case manager associate.
- Be a registered nurse and have a combination of specialized training in mental health, with three years of work experience consisting of community interaction and involvement or community discharge planning in a mental health setting.
- Valid Driver’s License and insurance for travel between sites and other related businesses.
- Must be able to pass a drug screening at hire and throughout the year.
